Abstract / Description
The goal of the PMW Foundation is to provide support, education, and outreach for veterans from the time of separation from active duty in military service. The Foundation provides:
- Benefits: Veterans’ benefits and opportunities via collaborations, partnerships, and direct and supplemental education options delivered in a culturally relevant manner.
- Healthcare: Healthcare navigation and resources include but are not limited to physical and mental health with a focus on the VA system and other venues with a focus on access, eligibility, and transition.
- Key to healthcare navigation is ongoing education with a focus on recommended screenings, risk assessments, tools for Veterans and their families, and a comprehensive overview of existing government and private recommendations and opportunities.
- Key to mental health navigation includes strategies to combat stigma, mentorship, and group collaborations and resources.
- Health and wellness plan implementation and intervention with a focus on prevention, early detection, and treatment for the individual Veteran and their families.
- Career & Lifestyle Strategies: Into civilian life that spans immediate separation from service forward into retirement and aged years.
- Families of Disabled or Deceased of Veterans: Support for families of disabled or deceased Veterans and the navigation of services with a focus on proper connections to existing and future programs. Other supportive and educational outreach in partnership with other institutions and organizations.
- AstraZeneca’s Accelerate Change Together (ACT) on Health Equity programs: Support for families of disabled or deceased Veterans and the navigation of services with a focus on proper connections to existing and future programs. Other supportive and educational outreach in partnership with other institutions and organizations.
